In clinical practice documentation is the primary method of communication regarding patient care. In a malpractice action it can be the basis for the plaintiff's case or for the health professional's defense. This practical video teaches health professionals to think defensively when charting patient records. Should a patient bring a malpractice case, a jury might not feel a procedure had been done if it was not written down. Records should include all diagnosis and treatment procedures.

The video highlights six steps that are essential to documentation:

  • Show how assessment of history and the physician was used in planning the patient's treatment
  • Document all patient protocols, along with parameters of standardized care
  • Note patient complaints that may be important and relevant. Records must show the rationale of providers for a particular procedure. List the patient's response to medication
  • Note if the patient consents or refuses certain treatments
  • Write legibly
  • Fill out all forms completely
